A quick thought
While completing an outpatient clerkship, I recently had a strange realization about all that was wrong with medicine at my preceptors office. A patient came in with a relatively simple abcess vs. cyst on the medial thigh. It was relatively superficial, not located near anything major, and my preceptor had extensive urgent care experience dealing with things just like this. In fact, I've done the I&D on similar lesions in medical school. So I asked, "are you going to drain that thing?" with of course the glimmer of hope that I might be able to do it.
He said no.
This patient's insurance wouldn't pay him to do it. As he put it, "I don't work for free." He instead spent 20 minutes on the phone referring to a general surgeon, getting approvals, etc... The patient's insurance was willing to pay a PCP bill, send the patient to a surgeon and pay that bill, and then have the patient return to both for more billing. What a bizarre system in which we send the patient to two extra appointments, pay for both, and waste the time of a highly qualified practicioner on the phone all for a simple procedure that the med student could have done and the doctor had done 1000 times before. You wonder why we spend so much money. There must be no real competition in the local insurance industry, because there is no way such a stupid system could survive any real competition.

As the fifth anniversary of the collapse of Lehman Brothers draws near and the debate about fiscal austerity continues to rage, it is time to take stock of the trajectory of debt levels in the key advanced and emerging market economies (AEs and EMs). The overall picture of government debt around the world is not a pretty one (interactive data here).
Data from the IMF’s latest Fiscal Monitor show that the level of aggregate net government debt in the world is expected to rise from $26tn in 2008 to $42tn in 2013. The ratio of world net debt to world GDP, a more relevant indicator of sustainable debt levels, shows a corresponding increase from 46 per cent in 2008 to 61 per cent in 2013.

Small Firm di London Atau Big Firm di Malaysia?
Lama benar rasanya aku tak update blog memandangkan sebulan lebih aku sibuk dengan urusan career. Kalau korang nak tahu, aku akan habis degree aku dalam Accounting and Finance di London School of Economics (LSE) Julai nanti. So, aku dah pun belikan ticket flight untuk mak aku dan kawan aku bagi membolehkan diorang datang Julai nanti. Ahakz!! Papepon aku minta maaf sesangat sebab lama tak update cerita baru kat korang semua yerk. Sepanjang sebulan lebih aku mendiamkan diri nie, banyak jugaklah interview yang aku pergi : accounting firms di UK dan juga di Malaysia. Aku sempat pergi interview dengan PwC UK, PwC Malaysia, Ernst and Young Malaysia, Flemmings UK dan juga beberapa firm lain. Memang banyak yang aku apply memandangkan aku plan nak buat Chartered Accountancy (CA) dan sponsor aku yakni PETRONAS pun highly recommend program nie tau (^_^)/

Untuk buat Program CA, aku perlu dapatkan placement kerja di accounting firm. Pastu aku kena bekerja dan juga belajar pada masa yang sama untuk dapat kelulusan CA nie. Selepas dah dapat CA, barulah aku akan kembali ke sponsor aku yakni PETRONAS semula. Faham korang semua?! *kih3!!* So, pendekkan cerita yang panjang, aku dah pun ditawarkan dengan dua placement kerja yakni Ernst & Young Malaysia dan Flemmings UK. Namun aku still tak dapat buat keputusan antara small firm di UK ataupun big firm di Malaysia. Kalau tanya aku directly, aku rasa nak balik Malaysia jer terus tapi mungkin kengkawan pembaca ada idea lain kan? So, aku sharekan details pasal kedua-dua firm nie kat bawah yerk!!

Flemmings UK – A Small Firm di Hujung London
Details : Firm nie kecil jer sizenya yakni dalam 50 to 70 orang pekerja dan ada cawangan di Chennai, India. Kebanyakan urusan dalam firm adalah menguruskan kewangan company kecil dan professional seperti doctor, dentist, lawyer dan sebagainya.
Working Life : Waktu bekerja adalah standard yakni dari pukul 8.15 pagi sampai 5.45 petang. Memang agak jarang untuk ada overtime. Pastu firm ada sediakan lunch vegetarian free setiap hari dan yang specialnya adalah korang tak boleh on handphone or tablet or ipad dan sewaktu dengannya langsung waktu kerja. Kerja sepenuhnya dan memang takda nak lepak-lepak or hang around sekeliling. Senyap : sunyi sepi. TT__TT
Contract : Contract kerja adalah 4 tahun dan nie buat aku rasa berat sikit. Gaji yang ditawarkan adalah dalam GBP 18,000 setahun yakni agak rendah berbanding gaji standard di UK. So, lepas tolak tax, bayar rumah dan sewaktu dengannya : maximum leh save adalah GBP 400 sebulan kot. Alright la kan?!

Ernst and Young Malaysia – A Big Firm di Kuala Lumpur
Details : Ernst and Young merupakan salah satu daripada empat accounting firm terbesar di dunia dan ada puluhan ribu pekerja dan cawangan di seluruh dunia. So, memandangkan nie antara the Big 4, maka clients pun besar-besarlah semua kan?!
Working Life : Aku rasa korang pun mesti dah familiar dengan working life di Malaysia kan? Kekadang kena bekerja sampai lewat malam kalau ada dateline. Bukan sahaja kerja sampai lewat malam tapi kekadang weekend pun angkut sekali. TT__TT
Contract : Berbanding dengan Flemmings UK yang contractnya adalah 4 tahun, Ernst and Young pula menawarkan kontrak hanya selama 3 tahun. Gaji starting adalah RM 2,800 dan insyAllah PETRONAS yakni my sponsor akan topup sikit lagi. Tapi hidup bujang cukuplah nie kan?!

So itulah sedikit sebanyak pasal dua tawaran kerja yang aku dapat; yang lain-lain aku dah tolak tepi awal-awal. Flemmings dah pun hantar formal letter tapi Ernst and Young still pending bab formal letter tu. Kalau ikutkan hati aku, aku lebih berminat untuk balik dan sambung kerja di Malaysia sebab kontrak kerja yang lagi pendek yakni hanya 3 tahun dan boleh belajar lagi banyak sebab Ernst and Young nie adalah firm yang amat besar. Tapi pada masa yang sama, kalau compare gaji secara direct, kerja di London lagi masyuk walaupun tak boleh guna phone langsung the whole time masa kerja. Lagipun kerja di London bolehlah kita balik on time kan? Ahakz!! Papepon aku nak tahu pendapat dan idea korang yerk. InsyAllah malam sebelum aku kena buat keputusan, aku akan sembahyang Istikharah untuk dapatkan petunjuk yang paling terbaik : yakni dari Allah SWT. (^_^)/
